申请条件：
申请人必须是ICOI研究员,包括能够植牙或做修复植牙的牙医,以及牙科技工。
Pre-requisite: Active ICOI Fellowship who restore implants or fabricate implant prostheses.

是ICOI Fellowship成员，必须表现积极活跃。在过去五年至少参加过一次ICOI/IPS主办的大会，或者是ICOI/IPS是主办方之一的大会。这样的会议包括：1.ICOI世界年会，2.ICOI亚太区年会，3.ICOI冬季讨论会，4.ICOI/IPS部门大会。
Active ICOI Fellowship. Must attend at least one ICOI/IPS sponsored or co-sponsored meetings in the preceding five (5) years. The meetings include：1. ICOI world Congress, 2. ICOI AP congress, 3. ICOI Winter Symposium and 4. ICOI/IP Section congress.

申请要求(5)：递交病例报告Provide a listing of completed implant cases
病例报告要包括至少12个月以前已完成的40个植体修复病例。将其中40个病例完整地填写在ICOI/IPS修复专业临床硕士病例文件表格中。
Provide a listing of forty (40) completed implant cases all of which must be at least 12 months old. Fully document forty (40) of these cases on ICOI’s Case Documentation Form B for Mastership and submit with the application. (See form for instructions). Further documentation may include patient slides or photographs, CT scans, pre-operative evaluation and planning forms,lab and restorative work authorization forms, and/or patient treatment consent forms, etc. to further detail a case. All materials may be submitted digitally.
牙医 Practitioner candidates：
在病例文件中要附上手术前和手术后的X光片副本。提供过去5年内至少完成了100个小时植体教育课时的证明文件。在ICOI/IPS国际口腔植体学会或植牙和假牙修复部举办或联合举办的座谈会上参加课程、讨论、演讲和临床示范将会得到4倍的课时奖励，而在所有非ICOI上可以得到2倍的课时奖励。
Provide documentation of completion of photographs or slides of completed cases on master casts or intra-orally are the minimum requirement for case documentation. At least one hundred (100) continuing implant education hours in the preceding five (5) years (either attending in person or completing courses on-line). These hours may also be attained by teaching courses or seminars or by giving lectures and table clinics using a 4:1 ratio at any ICOI or IPS sponsored or co-sponsored symposia. (i.e. two-hour lecture equals 8 hours of CE) or a 2:1 ratio for all non-ICOI symposia.
牙科技工Laboratory technician candidates:
在病例文件中要附上病例的主模型或口内照片或幻灯片。
photographs or slides of completed cases on master casts or intra-orally are the minimum requirement for case documentation.
申请要求(4)：递交临床示范或海报展示证明Submit evidence of table clinics or poster presentations
要出具在过去5年内在ICOI/IPS会议上至少做了2次临床示范或海报展示的证明。
Submit evidence of having presented at least two (2) table clinics or poster presentations at ICOI meetings within the last five (5) years
申请要求(5)：ICOI义务工作Serve on ICOI committee
已经为ICOI/IPS工作了至少一年的时间或愿意为ICOI进行义务工作。
Having served on an ICOI committee for a period of at least one (1) year or be willing to do so.
申请要求(6)：递交1封推荐信Provide a letter of recommendation
出具1封下列人员起草的推荐信，包括ICOI会员,研究员，修复硕士，院士，审查委员会成员或各大学及附属医院的牙科教师）
Provide a letter of recommendation from a current member, ICOI Fellow, IPS Master, ICOI Diplomate or member of ICOI's Advanced Credentials Committee and Dental university its hospital's faculty
修复硕士的责任Mastership Maintenance Requirements：
1.所有IPS修复专业硕士必须要有，并且每3年至少要参加1次ICOI/IPS或植牙和假牙修复部所举办或联合举办的会议。
Active ICOI Fellowship, all IPS Masters must maintain their membership in good standing (pay membersip due on time),and must attend at least one ICOI/IPS sponsored or co-sponsored meeting every three (3) years.
2.在成为IPS修复专业硕士后，所有修复专业硕士必须要在5年内累计接受100小时或更长时间的“植牙教育”。这些课时可以通过以下活动累计：
All IPS Masters must also accumulate one hundred (100) hours or more of "implant education" within five (5) years after becoming an IPS Master. These hours may be fulfilled through:
参加植牙座谈会。
Attendance at implant symposium
植牙演讲、讨论或者临床示范。在ICOI/IPS或植牙和假牙修复部举办或联合举办的座谈会上进行演讲、讨论或临床示范将会得到4倍的课时奖励(如2小时的演讲将可得到8小时进修教育课时)。而由非ICOI举办的活动则可以得到2倍的课时奖励。
Implant lectures, seminars or table clinics. Each lecture, seminar or table clinic given at ICOI or IPS sponsored or co-sponsored symposia will be credited on a 4:1 ratio (i.e. a two hour lecture will generate 8 hours of CE). A 2:1 ratio will be used for all non-ICOI programs.
在得到认可的期刊上发表植牙论文。在国际口腔植体学会出版物《口腔种植》和/或《口腔植体国际杂志》上发表1篇论文和/或病例报告将可得到20小时进修教育课时。每在一个认可的期刊上发表1篇植牙论文将可得到10小时进修教育课时。
Published implant articles in recognized journals. Each article and/or case report published in an ICOI publication, Implant Dentistry and/or the International Magazine of Oral Implantology , will be credited 20 hours of CE. Each implant published article in a recognized journal will be credited 10 hours of CE.
